Objective: To evaluate the effectiveness of Pilates exercise on improving balance, posture, and gross motor functional abilities in a child diagnosed with athetoid cerebral palsy. Method: An experimental case study was conducted on a single athetoid cerebral palsy child who met specific inclusion criteria. Pre-test values were recorded using the Pediatric Balance Scale (PBS), Posture and Postural Assessment Scale (PPAS), and Gross Motor Function Measure-88 (GMFM-88). The subject underwent a structured Pilates training program consisting of 12 exercises (supine, sitting, standing) for 8 weeks, 5 days per week, 45-60 minutes per session. Post-test values were collected after the intervention period and compared with baseline scores. Results: Post-intervention assessments revealed significant improvements in all outcome measures. There is a significant difference in pre- and post-test values, indicating better balance, enhanced gross motor function, and improved posture. Postural ability levels also improved across all evaluated positions (standing, sitting, supine, and prone). Conclusion: The findings demonstrate that Pilates is an effective therapeutic intervention for enhancing posture, balance, and gross motor function in children with athetoid cerebral palsy. It can be considered a promising addition to physiotherapy rehabilitation programs aimed at improving quality of life and functional independence in this population.
